How Did The Shootings Happen

Phoenix police responded to the home at about 2:15 a.m. Friday after receiving a report of a woman who was shot.

The shooter invited the first arriving officer inside and then ambushed him as he approached the door and shot him several times, Sgt. Williams said. The officer was able to get to safety as backup arrived and called out the occupants of the home.

Then, another man from inside the home came out holding a baby girl, Sgt. Williams said. He placed the baby on the ground, walked toward police and was detained.

As officers attempted to take the baby to safety, four officers were shot, Sgt. Williams said. Another four were injured by bullet shrapnel.

Two officers returned fire and the shooter then barricaded himself in the home, Sgt. Williams said.

The scene was ultimately secured, police said. The suspect was found dead inside the home.

A woman was also found injured in the home, Williams said. She died Friday afternoon, Sgt. Williams confirmed to The Arizona Republic.

The shooter was believed to be the woman’s former boyfriend, and the baby girl was believed to be their child, according to Sgt. Williams. He had no information as to whether police had responded to the home before.

The infant was in the Arizona Department of Child Safety’s custody as of Friday afternoon, Sgt. Williams said.

All five Phoenix police officers who were shot were taken to a hospital. There continued to be a large police presence outside Banner University Medical Center on Friday morning.

You can request to pick up your police report from the City of Phoenixs records office. Call ahead and check with a records clerk to determine whether your request is ready for pickup.

Requesting A Police Report In Person

1. Gather the required materials.

Bring with you a form of identification like a drivers license or passport. If you are making the request on behalf of someone else, you will need to have a notarized letter with you. Most agencies require a small request fee. How Do You File A Car Accident Report In Arizona

Under Arizona law, if you are a driver involved in an accident that causes injury or death, you must immediately report the accident to the local police department, county sheriffs office, or the nearest office of the state highway patrol. Another statute requires law enforcement personnel to complete a written accident report if they investigate an accident that causes death, injury or property damage in excess of $2,000.

At the minimum, an accident report will include:

  • The time and date of the accident.
  • The location of the accident.
  • Identifying information for all involved parties and witnesses, including name, age, sex, address, telephone number, vehicle ownership and registration and proof of insurance.
  • A narrative description of the facts of the accident, a simple diagram of the scene of the accident and the investigating officers name, agency and identification number.

The law enforcement agency that creates the report is then required to prepare the report within 24 hours and forward a copy of the report to the Arizona Department of Transportation.

Austin Michael, 26, used to live in a home that is rock-throwing distance from where the incident occurred Friday morning.

Michael had lived in the neighborhood for about a year and a half prior to moving out in September. During the past few days, hed been working on a car he bought from his former neighbors daughter, and opted to spend the night in the bed of his truck under a camper shell nearby.

He was on his phone around 2 a.m. when he saw the lights of a police cruiser pass by. Shortly afterward, he heard an exchange of gunfire. Additional police cruisers started arriving at the scene.

I got inside of my pickup truck, and I kept down low, he said, adding that he didnt want to get in officers way.

Michael said media personnel arrived and at one point were taking cover behind his truck when a round flew over their heads.

That was so close, thats when everything got extremely serious and it was not a joke anymore, he said.

Five Phoenix police officers were shot and injured, and a woman was shot and killed during an early morning barricade situation, the department said.

All of the officers’ injuries were non-life-threatening, police said.

The incident began when officers were called to a home where a woman was reported shot, Phoenix police said.

When an officer approached to help, the suspect, an adult man, invited the officer inside, said Phoenix police spokesman Andy Williams.

As the officer approached the door, “the suspect ambushed him with a gun and shot him several times,” Williams said. “That officer was able to get back and get away to safety.”

“Other backup officers arrived on scene and they surrounded the home and began calling out the occupants,” Williams said.

Then another man — not the suspect — came out of the house holding a baby girl, police said. The man put the baby on the ground and walked to police where he was detained.

When officers went to bring the baby to safety, the suspect again opened fire from inside the house, shooting four more officers.

Four other officers were indirectly injured when they were struck by shrapnel, police said.

The suspect then barricaded himself in the home, police said.

“He was compliant, detained and removed from harm’s way,” Williams said.

Obtaining A Copy Of A Car Accident Report In Arizona

In some cases, the responding police officer, sheriffs deputy or highway patrol trooper who responded to your accident will give you a copy of their report before departing the accident scene. If not, you should get the officers name and badge number and, if possible, the accident report number.

After several days, contact the local office of the law enforcement agency that handled your accident call and request a copy of the report from your car accident. There will likely be a small fee.

For example, you can obtain an accident report from the Maricopa County Sheriffs Office for $5 for the first 10 pages and $.50 for each additional page. It may be 7-14 business days before a report is available. The Department advises that reports may be delayed depending on the severity of the crash. Accidents involving serious and/or fatal injuries may take months to complete.
