If you elect NOT to use the portal, requests may still be submitted using U.S. Mail and email using the Incident Report Request Form. However, those requests will be manually entered and tracked in the Incident Report Request Portal by a member of CISU. You do NOT need to complete the Incident Report Request form if you submit your request via the portal.

How To Obtain A Car Accident Police Report

To get a police report after a car accident, youll need to bring a law enforcement officer to the scene if one is not already present. Once you and your passengers are in a safe location, call 911 to verify law enforcement and first responders are dispatched to the scene.

You are not legally required to talk to the police, but it may be in your interest to provide your side of the story for the police report. Do not admit fault, either accidentally or intentionally, and speak only about the facts of the incident.

Before the law enforcement officer leaves the scene, obtain their name, badge number and the police or incident report number if available.

Request A Police Incident Report Copy Online

Request and receive all types of San Francisco Police Incident Reports through an online portal. Report types include traffic collisions, domestic violence, and all other incidents of general crime. Using the portal is the most secure and convenient way to request and receive incident reports from the San Francisco Police Department. How To Request Records Contained In A Major Collision File

Major Collision Bureau investigates all Motor Vehicle Collisions resulting in a fatality or serious personal injury that occur in the Region of Peel.

  • Company letter or personal/company email requesting the records, including date, time, location, occurrence number, officer in charge of the collision. How you are involved or your relationship to the involved party.

    You may request a police incident report in person at SFPD Police Headquarters during our normal business hours of 8 am-5 pm, Monday – Friday .

    The SFPD Crime Information Services Unit is responsible for processing incident report requests from the public, law enforcement agencies and other authorized organizations. The unit also processes local criminal background checks for agencies entitled to that information under the law.     The process of requesting a copy of a police report by mail is a bit more involved. In many jurisdictions, you are required to make your Public Access Request via mail. Typically, the agency will provide a form online for you to fill out, but some places are fine with you writing a letter to ask for the police report.

    1. Figure out where to send the Public Access Request form or letter.

    Your local police department may have a specific address and addressee for police report requests listed on their website. If they dont, simply address it to the Records and Identification Division, and use the general address listed online for the department.

    2. Fill out the form or write the letter.

    After you have found the location to send your request, you should be able to easily find out if they want you to send a request form or a letter. If the information is not readily available on the site, just call and ask which method they prefer. If it is available, filling out a request form is typically the easiest method because you wont have to worry about leaving out information.

    If you write a letter instead of filling out the form, be sure to include the following information:

    Bay St. Louis Police Sgt. Steven Robin and officer Branden Estorffe spoke to veterinarian Amy Anderson for nearly a half-hour before she fired at them while sitting in a parked vehicle in a hotel parking lot.

    BAY ST. LOUIS, Miss. – A longtime veterinarian and mother has been identified as the woman who shot and killed two police officers in Mississippi on Wednesday, according to local reports.

    Amy Anderson, 43, of Ocean Springs, Mississippi, was sitting in a parked SUV with a female child younger than 10 years old when the officers were sent to the parking lot of a Motel 6 in Bay St. Louis around 4:30 a.m. on a welfare check, the Mississippi Department of Public Safety said in a statement.

    Bay St. Louis Police Sgt. Steven Robin and officer Branden Estorffe spoke to Anderson for nearly a half-hour before she fired at them while sitting in the vehicle, investigators said. During the conversation, the officers called for Child Protection Services, the police statement said.

    Robin, 34, died in the parking lot. Estorffe, 23, died a short time later at the hospital, Mississippi Bureau of Investigation agents said.

    It was still unclear what happened to the girl inside the SUV.

    “He knew it was his job to make great cops out of young rookies,” Schwartz said.
