Police Department Request Form

If you cannot submit a police report request online or through the mail, then you will have to visit the police department in person and file a request there. Call beforehand since some departments only allow in-person requests on certain days. You can find your local police department here. You may request the report in person, but reports take time to prepare and we strongly recommend that you call ahead for a copy of a report so that we can have it available to you when you come in.
