There is no nationally mandated format for police reports. Police departments in different jurisdictions use their own style of report forms, but forms in all jurisdictions contain similar information. A case number, or item number, typically is prominent on the form at or near the top of the form. This number may begin with the year, such as 2019 or simply 19, followed by a unique number thats assigned to each incident.

The content of police reports describes not only what the reporting officer saw at the scene of an incident, but also heard from victims, suspects and witnesses. Officers also may snap photos, sketch diagrams, and take measurements of distances or objects, which they attach to a police report.

Different sections of the police report include event information, which is what prompted the opening of a report, such as:

  • The date and time of an event.
  • The location of an event.
  • The reporting officers name, which also may include the officers badge number.
  • The victims personal information, such as name, address and physical characteristics.
  • The suspects personal information and physical description.
  • Offenses that may have occurred.
  • A listing of damages and/or injuries.

Purpose Of A Police Report

The purpose of police reports is not to issue charges for alleged crimes, which is the duty of a prosecutor such as a district attorney or judge. Its only after a prosecutor issues charges to an alleged offender that a court case is opened. This is one example of when a police report enters the judicial picture another example is when a person chooses to bring a lawsuit against someone in a civil matter.

Although police officers investigate crimes, a police report doesnt drive the judicial process in either of these two examples. Instead, it serves as an unbiased account by law enforcement of the details surrounding an incident.

Reasons For Police Report Delays

If you didn’t get a police report or incident number at the site of your incident, it can make it difficult to check status, specially if you are trying to do this online. A station clerk or officer may be able to locate the status of your report with the date/time and location of the incident. Not having your report or incident number can also delay receipt of your report when requesting a copy. Sometimes police reports are delayed because the officer has yet to process the paperwork, or there is a mistake with the paperwork that must be corrected before generating a report. How To Access Your Police

Members of the public have the right to request records containing their personal information from the Ontario Provincial Police or a municipal police service. Individuals who make these requests may have, for example, played a role in a suspected offence, been involved in a call for service or some other police-attended incident or had a relative who died in circumstances that led to the involvement of a police service.

Someone else, such as a lawyer or other person acting on your behalf, may make the request for you, or you may do so yourself. You may file a request by submitting a form and paying a fee, either by mail or in person.

Commonly requested records include:

  • officers notes and
  • police-related 911 calls.

Many people think that they cannot request a police officers notes, but a police notebook is generally in the custody or control of the police service and is subject to freedom of information requests.

Requesting information from a police service follows the same process as asking for records from other types of government institutions. However, the nature of police-held information means that frequently some of these records or some passages within these records are exempt from disclosure under Ontarios access to information legislation.

Police report summaries are available to authorized parties in person at the Police Department, or by mail. Reports are not available by fax. To obtain a summary of a police report , you must be authorized to receive the report. Authorized persons include:

  • You must be a named involved party to the case . You must be: A victim, an involved named party, an involved driver, or
  • An attorney, authorized agent, or insurance representative for one of the victims or named parties .
  • Any persons named as a suspect may not get copies of reports from the police department. Anyone named as a suspect should call the San Mateo County District Attorney’s Office, 650-363-4677, or have their attorney call for more information.
  • For traffic accident reports, copies may be available through the Crashdocs website at or your insurance representative/agent. If not, you may obtain a copy from our Department.

How To Find Police Reports And Arrest Records Across Multiple Jurisdictions

Requesting A Police Report By Mail

The process of requesting a copy of a police report by mail is a bit more involved. In many jurisdictions, you are required to make your Public Access Request via mail. Typically, the agency will provide a form online for you to fill out, but some places are fine with you writing a letter to ask for the police report.

1. Figure out where to send the Public Access Request form or letter.

Your local police department may have a specific address and addressee for police report requests listed on their website. If they dont, simply address it to the Records and Identification Division, and use the general address listed online for the department.

2. Fill out the form or write the letter.

After you have found the location to send your request, you should be able to easily find out if they want you to send a request form or a letter. If the information is not readily available on the site, just call and ask which method they prefer. If it is available, filling out a request form is typically the easiest method because you wont have to worry about leaving out information.

If you write a letter instead of filling out the form, be sure to include the following information:

  • Case number
  • Your Name, Birthdate, and Address
  • The date and location of the incident
  • Your role in the case
  • The officers name who filed the report
  • What exactly you are requesting

3. Gather additional materials and send the form/letter.

4. Pay for the police report.

Why Are Some Things Blacked Out On The Page

This is called redacting and it is done to protect the privacy of the people involved in the incident.

The Department redacts the names of all victims, all non-involved parties, all juveniles/minors and all suspects unless they have been booked into jail. This is a major reason why it takes time to prepare crime reports for online posting.

Once a suspect has been booked into jail, their name is no longer private. If they were booked into King County Jail you can view suspect information online at the King County Jail Register.

Requests for copies of an MV-104A for collisions investigated by the New York State Police that occurred anywhere in New York State on roads or highways other than the Thruway must be made to DMV.

The form required to request a copy of the report is available online at: www.dmv.ny.gov.

Note: Although all Police Accident Reports completed by the New York State Police are submitted electronically and are generally available within thirty days, there are several variables that affect the time in which the report is available to the public. Consequently, some reports may be available within three weeks while others may take as long as six weeks to process.

    Washington Township Is Accepting Applications For A Police Records Clerk Through Feb 21

    LONG VALLEY, NJ The police department is seeking a records clerk, according to the township of Washington.

    Applications are currently being accepted for the position through Feb. 21, either full-time or part-time, with the salary range between $18 and $23 per hour.

    A prospective candidate will support the police department with customer service, organizational and computer skills, according to the township. In the role, the police records clerk will also be expected to interact with police officers and the public, as well as county, state and federal agencies.

    Hard Copy Police Reports Available

    Reports are typically available for pickup four business days after the report was taken. To get the status of a police report, please call 770.513.5000.

    Incident and accident reports may be obtained at any of the 7 locations listed below. Reports are available Monday through Friday. Offices/Precincts are closed on county holidays.     LexisNexis, a commercial vendor, provides police reports for vehicular accidents. Police departments that use this service are able to capture crash data at the scene, and a full report is then available to permitted persons within 24 hours after law enforcement generates the report. This service is generally only available to named parties, that is, the persons who were involved in the accident, as well as commercial account holders, for example, lawyers and the the police department itself. To search for your report, enter your state and jurisdiction, and any of the following:

    • Full name and date of incident
    • Full name and address
    • Report number

    Not all police departments make their reports available through third-party vendors. The police department, rather than LexisNexis, sets the fee for the report, but you remit payment to LexisNexis.

    After you have been in a car accident, one of the first questions your insurance provider will ask you is do you have a copy of the police report? Obtaining accident reports is a similar process to the one above, but it is generally a bit easier because in most cases, there is no criminal investigation. If you are in a car accident, you will receive a slip with an incident number on it. To obtain your police report, simply visit the police department that the report was filed from and trade the sheet for the actual report.     What Information Does A Police Report Contain

    This document is an official report by the officer which includes all relevant details related to the accident, and may even include photographs depending upon the severity of the accident.

    The police report will include such things as the time of accident, who was involved in the accident, where the accident occurred, and any damages that may have resulted.

    It will also include the officers observations, including whether he or she issued a citation, found that one party may be at fault, and any other factors involved in the accident.

    This report is absolutely vital, especially if you will be seeking a claim from an insurance company. Just having a document from the other driver explaining what happened may not be solid enough evidence in the view of your insurance company.

    Having that police report to support any assertions you are making really bolsters your claims before the insurance provider.

    California Family Code § 6228 provides that victims of the alleged crimes, listed above, may receive a copy of their incident report within five days of their request, free of charge. Due to the sensitive information included in the reports, SFPD protocol requires in-person identification verification. Victims or their designated representatives must either submit their request in-person or pick-up their incident report in person at CISU Records Management. For a list of acceptable forms of identification, please refer to Section 2 of the Request for Police Report: Family Code § 6228 form.
