Police Incident Report Copy Request Forms
If you elect NOT to use the portal, requests may still be submitted using U.S. Mail and email using the Incident Report Request Form. However, those requests will be manually entered and tracked in the Incident Report Request Portal by a member of CISU. You do NOT need to complete the Incident Report Request form if you submit your request via the portal.
Our request forms are available in several languages. Select your language, download and fill out the request form in your preferred language. The completed form may be either mailed or emailed to SFPD for processing. Instructions are included on all forms.
You Cannot File A Report Online If
- A threat was made or violence was used during the incident. If you are in immediate danger, please dial 911. If you are no longer in danger, please call us at 613-236-1222, extension 7300.
- Evidence was left on the scene such as, but not limited to blood, tools, clothes or any item related to the incident. Please call 613-236-1222, extension 7300 to file a report.
- The incident being reported did not happen in Ottawa. Please contact the police service in the city where the incident happened to file a report.
- The stolen property is a passport
- The property lost is a cell phone, licence plate, bank card or government issued identification, with or without a serial number. Please contact your service provider to get a replacement.

Obtaining A Physical Copy Of Your Accident Report
You can also visit the respective police department and pick up a copy of your report. The Indiana state government imposes a fee of $12 for the copy.
You can also obtain a copy of an accident report made by an Indiana Metropolitan Police Department officer by visiting the following address: 50 N. Alabama St., E100l, Indianapolis, IN 46204. The office is open between 8 am and 4 pm. As part of the processing, you are required to provide the date, time, and location of the accident. Traffic Collision Reports: Before attempting to obtain a copy of a collision report, please allow a minimum of five days from the date of the collision to allow the report to be reviewed and approved for release.
What Type Of Information And Data Is Contained In A Police Report

Submit Your Car Accident Report To Dmv
Most states require submitting an accident report to the DMV. Oregon residents, for example, must file these forms within 72 hours of the accident, provided there’s substantial damage and/or serious injury involved. California’s DMV requires something similar, but within 10 days for any accident resulting in death, injury , and/or property damage of $750 or more.

To see if your report is available in our system use ‘List Reports on File by Date’ to see a list of accidents by the county and date you select . The list of accidents will include the plate numbers of the vehicles involved in the accident.
If there are reports you want, you will need to do a paid search before you can order them.
Second: Do a ‘Paid’ search
To search for your report use ‘Search by Plate Number, License Number, or DMV Accident Case Number’ .
You will be charged a fee even if
- you enter the incorrect information, or
- if the report are not found

How To Access Your Police
Members of the public have the right to request records containing their personal information from the Ontario Provincial Police or a municipal police service. Many people think that they cannot request a police officers notes, but a police notebook is generally in the custody or control of the police service and is subject to freedom of information requests.
