You can upload this in the country-specific police certificate field of your document checklist. Examples of proof:

Verification of Incident reports are provided by the Criminal Records Section, which stores and maintains crime and lost property reports and provides information from these files to members of the public and authorized agencies, as required by law and department regulations.

You can request a copy of a Verification of Incident by submitting your request online or by mail. The form is available for , or can be picked up at any police precinct, Housing Bureau public service area, or Transit Bureau district. Reports can only be returned through the mail and are NOT available at Police Headquarters.

If mailed, you must include two copies of the completed form, along with a self-addressed stamped envelope. Complainants/victims designating an authorized representative must complete, notarize, and mail the request to the address above.

For the foreseeable future, to obtain an accident or incident report from our Records Management, please follow the below steps:

  • To have a copy of an accident report or incident report emailed to you or sent via United States Postal Service
  • Email or depending on which report you need
  • Include in your email, the report number, if you have it your name name of involved, if not yourself, and your relationship to them date occurred and location occurred.

    If the report contains information involving a juvenile you must complete a “Request for Release of Juvenile Case Information” form. You must appear in person at the police station to execute this document.     The majority of requests are traffic accident reports, or incident reports. Crime Reports include all reports taken by the police department, in person or by telephone, other than a traffic accident.

    How To Get A Local Police Records Check

    Fill in all questions on page 1 of the Local Police Records Check Form.

    Tip: Photocopy it for each local police service you contact.

    Contact the local police service for the address where you live now. If you have lived in different places during the last 5 years, you must contact the police service for each place. If you are not sure who to contact, ask your local police service.

    If you lived outside of Canada you still need to contact the local police service where you lived. If the police service outside Canada will not give you a local police records check, a signed letter from the police service stating that you have been law-abiding will be accepted. If it is in a foreign language, you must have it translated into English or French and submit both the original and the translated version with your application form.

    Show your criminal record to each local police service and ask them to fill in the section on the Local Police Records Check Form called For Police Use Only on page 2 of the form.
