COST: Cash, check, money order for $10.00, payable to UC Regents, must be paid at the time the request is submitted.

Once payment is received, reports can be scanned and sent electronically as a PDF. Reports can also be sent by fax or U.S. Postal Service mail. On the Form remember to include your mailing address, email address and/or fax number. If you do not have the case number, the records manager can retrieve the case number for you.

    You aren’t required to explain why you are making a request.
