* What Is A Qualified Requester
A qualified requester is a person involved in an incident or accident as either a victim, offender, driver, vehicle owner, involved pedestrian, or the owner of damaged or stolen property. Anonymous callers and uninvolved 3rd parties may receive police reports in redacted form only in order to protect the identity and privacy rights of those involved.
A signed authorization from a qualified requester allows MPD to release a specific report to an authorized representative or agency. This authorization must be accompanied by a photocopy of the qualified requesters photo I.D.
Qualified requesters or authorized representatives may receive either preliminary reports or complete police reports depending on the severity and prosecution status of the associated criminal matter. The Maui Police Department will release and/or withhold all necessary information in accordance with the Hawaii Uniform Information Practices Act Hawaii Revised Statutes §92F
Review Of The Report By The Prosecutor
The written report is sent to the prosecutor, who must then decide if more investigation is necessary or if enough evidence exists to authorize prosecution based on probable cause that a crime has occurred. The evidence must also support that the person cited in the report committed the crime.
The American Bar Association notes that this should not be confused with proving guilt beyond a reasonable doubt, which is a heavier burden to meet and is required for conviction at trial.
Request A Police Incident Report Copy
Effective Tuesday, July 6, 2021, SFPD Police Headquarters at 1245 Third Street will reopen to the public after a temporary closure due to the COVID pandemic.
You may request a police incident report in person at SFPD Police Headquarters during our normal business hours of 8 am-5 pm, Monday – Friday .
The SFPD Crime Information Services Unit is responsible for processing incident report requests from the public, law enforcement agencies and other authorized organizations. The unit also processes local criminal background checks for agencies entitled to that information under the law. For all other requests that are not incident reports or local background checks, please contact the appropriate department within SFPD.

How Can I Get My Nypd Accident Report In Person
If you want to pick up your NYPD accident report in person, you must fill out two copies of a Police Collision Report and drop them off at the precinct where your crash occurred. Keep in mind, NYPD precincts only hold on to accident reports for 30 days after the officer files them. Afterward, they send the report to the New York State Commissioner of Motor Vehicles.
Upon Completion Of This Report Process You Will:
- See the words: “Your online police report has been submitted”.
- You will then immediately receive a temporary police report file number and an email informing you that we have received your report and that it is pending review. A temporary file number will appear in this format: T11000999. Temporary file numbers cannot be used for insurance purposes.
- If you do not receive an email with a temporary file number, please check your Spam or Junk-Email folder.
- A police officer will review your report and either Approve, Reject or request additional follow-up information from you. You will be advised via email of the status of your report. Do not reply via email to any of these automated messages as these accounts are not monitored. Follow the directions provided on how to respond regarding your report.
- Once your report is approved, you will receive a permanent report number – this number appears in this format: 11-123456.
- Be able to print a copy of the police report to keep for your records only. This copy cannot be used for insurance purposes.

What To Do If You Do Not Have The Receipt
Sometimes, people need a Police Accident Report and they do not have the Incident Report. For example, if you are a passenger or pedestrian that was involved in an accident, it is quite likely that the responding officers did not provide you with any documentation.
If you do not have the receipt, give the clerk:
- Your full name
- Approximate time and location of the accident
- A license plate number of at least one of the involved vehicles
The clerk should be able to use this information to retrieve your case details.

How To Get A Police Report For A Car Accident
After you have been in a car accident, one of the first questions your insurance provider will ask you is do you have a copy of the police report? Obtaining accident reports is a similar process to the one above, but it is generally a bit easier because in most cases, there is no criminal investigation. If you are in a car accident, you will receive a slip with an incident number on it. To obtain your police report, simply visit the police department that the report was filed from and trade the sheet for the actual report. If you do not obtain your police report, your insurance company will have to request a copy via mail, and it will take a bit longer.

Why Do I Need A Police Report
The crash report for your accident will contain crucial information about your case. The police take the time to investigate the accident, inspect the vehicles, talk to witnesses, and take photographs of the scene. All this information will be included in the report, including the following information:
- An approximate date, time, and location of the accident

However, those requests will be manually entered and tracked in the Incident Report Request Portal by a member of CISU.
