File A Police Report At The Scene
If you are the victim of or witness to a serious crime or a car accident where a police officer is called to the scene, you might file a police report at the time of the incident. If you later need to obtain a copy of the report for any reason, you can request the report either in person at Central Police Headquarters at 3510 S. Michigan Ave., or by mail.
Mail report requests to the Chicago Police Department, Records Inquiry and Customer Service Section, Unit 163, Chicago, IL 60653. Cook County does not provide online access to criminal records. If your case was filed in Cook County, go to the Circuit Clerk‘s Office in the district where you went to court. There are no court dispositions for arrests or charges that did not lead to a court case. In certain courthouses, the Clerk may charge you a Certified Disposition Fee to process your request. If youd like to access your criminal court records in the Circuit Court of Cook County, you must use a public-access terminal at a Cook County courthouse since the system does not publish criminal records online. You can purchase a certified disposition of your case directly from the circuit court for $9.

To obtain the required form, that individual would access the Illinois State Police website, and the form is found under the “Criminal History” tab, and is labeled as the Uniform Conviction Information Form. There are two forms to chose from: the Non-Fingerprint Request Form , and the Fingerprint Request Form . The Non-fingerprint request form performs a search based on alphanumeric identifiers such as name, sex, date of birth, and race. The Fingerprint Request Form performs a positive identification match based on the fingerprints contained on the Request Form. Individuals with an older or more extensive criminal history may prefer to utilize the Fingerprint Request Form, especially if the individual has changed their name, or if a conviction was under a maiden name or alias, because it will provide more accurate information and will lessen the likelihood that the individual’s criminal history report will show inaccurate or incomplete information. If the Fingerprint Request Form is used, the individual would need to visit a live scan fingerprinting location to have their fingerprints taken. The individual would then request that the live scan request form have the same Transaction Control Number as was issued to them on the Fingerprint Request Form by the Illinois State Police.

I do not want the arresting officer to know that you have an attorney and are getting ready to fight the case. This will keep the case fresh in the police officers mind.

Uniform Conviction Information Form
Once an individual receives the Uniform Conviction Information Form, it must be filled out completely and submitted to the Illinois State Police’s Bureau of Identification. Both forms require that a fee be submitted along with the form for the service. The Non-Fingerprint Request Form carries a fee of $16.00, and the Fingerprint Request Form carries a fee of $20.00. The fee must be paid by check or money order.
