After submitting the report, a temporary number will be automatically sent to you by email. All communication will be done by email. Your report will be reviewed and if any changes are required, you will be sent a request for changes. Once the report is corrected and approved, you will receive an official event number by email, as well as a PDF copy of the report.

Make sure you have a valid email address as well as the date and location of the crime and information on the vehicle involved, if applicable.

If you meet these criteria and the type of incident that occurred appears on the list above, you may file your report.

To report a theft or loss of packages, please note that some companies require a report in English only. We offer the service in the language of your choice. Note, however, that once the report has been submitted, it is no longer possible to change the language.

How To Request Records Contained In A Major Collision File

Major Collision Bureau investigates all Motor Vehicle Collisions resulting in a fatality or serious personal injury that occur in the Region of Peel.

  • Company letter or personal/company email requesting the records, including date, time, location, occurrence number, officer in charge of the collision. How you are involved or your relationship to the involved party.
  • Clients signed consent or two copies of your government issued identification and your contact information.
  • Our office will advise you in writing regarding the status of the case or any known court dates.
  • If you are NOTthe Involved Party to the Collision

    You must provide proof of relationship to the involved party and a signed consent on behalf of the involved individual.

    If the Involved Individual is Deceased

    You are required to provide an authorization from the legal representative or power of attorney with supporting documents.

    How To Make A Freedom Of Information Request

    Copy Of A Police Report

    The VPD Correspondence Unit can provide a copy of a police report to you if you have reported or been involved in a property offence incident, such as:

    • theft
    • break-and-enter
    • motor vehicle collisions where a police officer has completed an MV6020 report

    With your consent, your insurance company can make this request on your behalf. In most cases, your police report number is sufficient for the VPD to find your file.

    What Happens After I File A Report Online

    • Upon submitting your completed report online, you will see the words Your online police report has been submitted indicating your report is complete.
    • You will receive an email with a temporary case number until police have reviewed the report. Check your junk email folder if you do not receive the email in your inbox.
    • Once a police officer has approved the online report, you will be notified and provided a permanent case number.
    • The accuracy of the information you provide in the report greatly assists Peel Regional Police Service in investigating your incident. Please ensure the report is as complete and concise as possible.

    Request Forms For Ca Family Code 6:

    Victims of Domestic Violence, Sexual Assault, Stalking, Human Trafficking, or Abuse of an Elder or Dependent Adult

    California Family Code § 6228 provides that victims of the alleged crimes, listed above, may receive a copy of their incident report within five days of their request, free of charge. Due to the sensitive information included in the reports, SFPD protocol requires in-person identification verification. Victims or their designated representatives must either submit their request in-person or pick-up their incident report in person at CISU Records Management. For a list of acceptable forms of identification, please refer to Section 2 of the Request for Police Report: Family Code § 6228 form.

    How To Get A Police Report For A Car Accident

    After you have been in a car accident, one of the first questions your insurance provider will ask you is do you have a copy of the police report? Obtaining accident reports is a similar process to the one above, but it is generally a bit easier because in most cases, there is no criminal investigation. If you are in a car accident, you will receive a slip with an incident number on it. To obtain your police report, simply visit the police department that the report was filed from and trade the sheet for the actual report. If you do not obtain your police report, your insurance company will have to request a copy via mail, and it will take a bit longer.

    There are also resources online that house accident reports, like LexisNexis and Buycrash. However, the safest and quickest option to is visit your local Police Desk as soon as possible after the accident. Accident reports can be ready to pick-up as early as 24 hours after the accident, and up to a week. Another way to get a copy of a police report is through your local Department of Motor Vehicles.

    Crimes Not In Progress

    Depending on where the crime occurred, you may report the crime to agencies such as the MTA police, NY State Police, or Port Authority Police, if the location of the crime falls within their respective jurisdictions. Otherwise, for certain report types, you can file an online police report or you can visit your local precinct.

    Has someone damaged, defaced, or vandalized your vehicle? This might include:

    • Denting, scratching or keying the vehicle
    • Throwing eggs or another object at the vehicle
    • Breaking windows or lights

    Please note: If the theft is still in progress, or you know the location or identity of the suspect, call 9-1-1. Gas thefts can also be reported in Chinese, Farsi, Italian and Russian.

    Requirements For Filing An Online Report

    Crimes filed online often dont need in-person police response. To file online, the incident has to meet the following criteria:

    • Its not an emergency
    • Theres no known information or suspects about the crime that can be followed up on
    • You must have a return email address
    • You must be 18 years or older

    Ensure you have the following details to submit your report:

    • Date and time when the incident occurred
    • Location, address, and name of the business where the incident happened
    • Your name, date of birth and contact information
    • The name, date of birth and contact information of other persons involved in the incident
    • If the report involves property, includes make, description, serial number, model and estimated property value

    How Can You File A Police Report For Stolen Property

    To file a police report for a stolen item, first make contact with the police department. Then provide the additional information the police needs to locate the item. Detailed information helps the police, and a thorough report makes insurance claims easier to navigate.

  • Contact the local police department

    To report an item as stolen, call the police department as soon as possible. Some jurisdictions have a non-emergency number to be used for cases such as stolen property.     The following are types of data and information that may be included. Please note that there is no universal police report. Information will vary from department to department.

    • Identifying information for all parties involved in the incident, including full name, address, phone number, date of birth, social security numbers, driver license numbers

    • The officers name and ID number
    • The names of other officers who were present
    • Diagrams or drawings of the scene

    The law enforcement case file may also contain motor vehicle information, as well as specific details of the incident and any evidence associated with the case.

    Examples of police reports may include but are not limited to domestic disputes, traffic accidents, thefts, fraud, assault, burglaries, and stalking incidents.     Visit a Calgary Police Service location and remember to bring:

  • Two valid pieces of government issued identification

  • Drivers Licence

    What Information Is Contained In A Police Report

    If youve ever been involved in a car accident, youve likely seen the responding officer walking around the scene of the accident taking notes. These notes are used in the police report, which, for a car accident, contain some or all of the following information:

    • Name of the responding police officer
    • Approximate date, time, and location of the accident
    • Name and contact information of the parties involved in the accident
    • Name and contact information of any witnesses to the accident
