- Your report number
- The report date
Access your report online. If you have technical issues call any time at 866-495-4206, option 5.
If you do not know your incident report number, you can email us. We will look up your information and email it back to you.
What If You Dont Know Which Law Enforcement Agency Responded To The Crash
This is common when someone is shaken after an accident. Its easy for the entire experience to become a blur. But that doesnt mean you cant get your report.
If you want to track down the agency, it will either be the police, sheriff, or Highway Patrol for the location where the crash happened. You can call each of those three agencies separately and ask them, and typically find out who has your report in an hour or less. Alternatively, you can use the website above to look up your report by location, date, and VIN number . The website does not require you to know which law enforcement agency responded.
Why Police Reports Matter
Police reports are public records and in some states they may be viewable by just about anybody. Usually there are restrictions on how those records can be used, particularly by employers, landlords, and financial institutions. As already mentioned, police reports are particularly usefuland they may even be necessarywhen filing a claim with your insurer. The contents of a police report are important, so you need to make sure that any police report pertaining to you is accurate and does not cast you in an unnecessarily bad light.

How Do I Obtain A Police Report
In most cases the public portion of police reports can be obtained from the APD Incident Report Database. Please read the Incident Report Database instructions on how to obtain the report. Make sure to enter the case number exactly how it is shown in the instructions.
For information about purchasing a copy of the public portion of a police report contact the Austin Police Department Report Sales Unit at 974-5499. Reports can be purchased at the Main Police Headquarters, located at 715 E 8th Street. Reports are 10 cents a page and only the public portion is available for purchase from the report sales office. This is the same report that can be obtained from visiting the APD Incident Report Database. Report Sales new hours of operation are Monday through Friday, 8:30 a.m. â 4 p.m.

Request Forms For Ca Family Code :
Victims of Domestic Violence, Sexual Assault, Stalking, Human Trafficking, or Abuse of an Elder or Dependent Adult
California Family Code § 6228 provides that victims of the alleged crimes, listed above, may receive a copy of their incident report within five days of their request, free of charge. Due to the sensitive information included in the reports, SFPD protocol requires in-person identification verification. Victims or their designated representatives must either submit their request in-person or pick-up their incident report in person at CISU Records Management. For a list of acceptable forms of identification, please refer to Section 2 of the Request for Police Report: Family Code § 6228 form.

Can I Get A Copy Of A Police Report For Insurance Purposes
Yes. Your Police Acknowledgement Form will be emailed or posted to you.
If you reported online via 105.police.govt.nz you should receive your PAF within 24 hours of making the online report.
After you have made a report to Police and been given a reference number, you can use the Case / Report update form available at 105.police.govt.nz to:
- provide additional information
- request an update from Police about your case / report
- withdraw your report.
Police Incident Report Copy Request Forms
If you elect NOT to use the portal, requests may still be submitted using U.S. Mail and email using the Incident Report Request Form. However, those requests will be manually entered and tracked in the Incident Report Request Portal by a member of CISU. You do NOT need to complete the Incident Report Request form if you submit your request via the portal.
Our request forms are available in several languages. Select your language, download and fill out the request form in your preferred language. The completed form may be either mailed or emailed to SFPD for processing. Instructions are included on all forms.

Police Department Request Form
If you cannot submit a police report request online or through the mail, then you will have to visit the police department in person and file a request there. Call beforehand since some departments only allow in-person requests on certain days. You can find your local police department here. Then you can find out about filling out a Police Department Request Form in person and how to get the information you need. Typically these police department request forms can be fulfilled that day.
A request will typically require you to provide your personal and contact information as well as details about the incident, including the type of incident, date, and who else was involved. Be aware that if the report pertains to an ongoing investigation then your request could be denied. If you are unclear about why your request was denied you should talk to an attorney.
If The Police Do Not Come To The Accident
There may be times that a police officer cant come to the scene of the accident. Here are a few reasons why that might happen:
- In some states, the police are not required to go to the scene of an accident if the property damage value is too low. If you are concerned about injuries or other issues, make the police aware when you call it in. Having more details can help them decide whether they will come.
- The police may decide not to come if there are no injuries.
- If there is a public emergency or disaster underway, they may be too busy to make it to the scene of your crash.
You should always make a call to the police and let them decide whether theyll come out. If the police do not come to the scene, then make sure you record all the details of the accident and any damages yourself.

Obtain A Public Police Report Copy
After filing a public police report you are able to obtain a copy for yourself any time. Unless you received one from a dispatched officer on the scene, you most likely will have to pay a fee to obtain a copy.
Most police reports are public record so anyone who wants to see their own or someone elses report is able to do so. Reports that are under investigation though are usually restricted to the police department and investigating parties, however.
How To Order And Access Motor Vehicle Accident Reports
DMV keeps accident reports for 4 years from the date of an accident, except for rare cases. 1

You must pay both a

First: Do a Free limited search to see if the DMV has your report
To see if your report is available in our system use List Reports on File by Date to see a list of accidents by the county and date you select . The list of accidents will include the plate numbers of the vehicles involved in the accident.
If there are reports you want, you will need to do a paid search before you can order them.
Second: Do a Paid search
To search for your report use Search by Plate Number, License Number, or DMV Accident Case Number .
You will be charged a fee even if
- you enter the incorrect information, or
- if the report are not found
Each search costs $7.
To order reports, you will need one of the following
- the plate number of a vehicle involved in the crash/accident, and/or
- the driver license number of a person who was involved in the accident, and/or
- the 8-digit DMV case number .

What Is A Car Accident Police Report

Third Parties Obtaining Reports

What Type Of Information And Data Is Contained In A Police Report
The following are types of data and information that may be included. Please note that there is no universal police report. Information will vary from department to department.
- Identifying information for all parties involved in the incident, including full name, address, phone number, date of birth, social security numbers, drivers license numbers
- Date of the occurrence or incident
- Location of the occurrence or incident
- The officers name and ID number
- The names of other officers who were present
- Diagrams or drawings of the scene

You may request a police incident report in person at SFPD Police Headquarters during our normal business hours of 8 am-5 pm, Monday – Friday .
The SFPD Crime Information Services Unit is responsible for processing incident report requests from the public, law enforcement agencies and other authorized organizations. The unit also processes local criminal background checks for agencies entitled to that information under the law. Are Police Reports Admissible In Court
While police reports are commonly used in car insurance settlement negotiations, admitting a police report as evidence when you file a car accident lawsuit is not quite as clear cut.
In small claims courts, litigants are usually permitted to use police reports as evidence in their car accident case. You will not be expected to know all the rules of evidence, so judges typically will allow plaintiffs and defendants to use the police report in explaining what happened.
If your car accident case goes to trial in your state’s court of general jurisdiction , you should know that parties in these cases are held to the rules of evidence and must contend with whether the police report falls within the rule against “hearsay” evidence, which keeps out many out-of-court statementsby definition, any assertion made in a police report is a statement that was made out of court.
