The VPD Correspondence Unit can provide a copy of a police report to you if you have reported or been involved in a property offence incident, such as:
- motor vehicle collisions where a police officer has completed an MV6020 report
With your consent, your insurance company can make this request on your behalf. In most cases, your police report number is sufficient for the VPD to find your file.
How To Request A Police Report Online

Many police departments now allow you to request your police report online, although some may require you to print off a police report request form and mail it to them with a self-addressed stamped envelope. San Antonio, for example, allows you to submit a request for a police report online, in person, or through the mail.

Submit Your Car Accident Report To Dmv
Most states require submitting an accident report to the DMV. Oregon residents, for example, must file these forms within 72 hours of the accident, provided there’s substantial damage and/or serious injury involved. California’s DMV requires something similar, but within 10 days for any accident resulting in death, injury , and/or property damage of $750 or more.
Inquire with your DMV if you’re unsure this requirement applies to you. Failing to comply could result in the loss of driving privileges. Or worse. In New Hampshire failing to submit a car accident report involving death or personal injury is considered a felony.
Car wrecks are traumatic enough. Don’t suffer additional penalties from ignorance of the law. Be certain. Call your DMV for its car accident policies.

Requesting A Police Report In Person
1. Gather the required materials.
Bring with you a form of identification like a drivers license or passport. If you are making the request on behalf of someone else, you will need to have a notarized letter with you. Most agencies require a small request fee. If you were directly involved in the incident whether as a victim, witness, or defendant you may receive a verification slip from the authorities to let you know that a police report has been filed. Bring this slip with you to exchange for the police report.
2. Visit the Records and Identification Division of the police department.
The Records and Identification Division may have exclusive hours throughout the day that are not your typical business hours, so be sure to verify online. They will ask you for your information, as well as basic information about the case. Then, they will take your payment to cover the request fee.
3. Retrieve the copy of your police report.
In some areas, you may have to pick up a copy of the police report in person, but some agencies may mail it directly to you. Either way, expect to pay a per-page fee for the copy of the report. Reports typically cost a total of $50 to $150.

Police Report Vs Police Record
First, lets clarify something that is bound to cause some confusion: the difference between a police report and a police record. While these two items have similar names, they are in fact very different. A police report is a document filed by a police officer that essentially offers a summary of an arrest, incident, or accident. A police record, on the other hand, is usually the same as a criminal record and it includes a list and summary of an individuals history of interactions with law enforcement, including arrests and convictions.
Another point that deserves clarification is the difference between a police report and an accident report. In fact, theres not really a difference between these two items. An accident report is simply a type of police report that details the circumstances of a traffic accident. A police report, meanwhile, can include accident reports, but they cover a much broader range of incidents that may involve the police, including crimes. Note that some police departments call police reports incident reports. Remember that some accident reports may also be criminal reports, such as if the accident may have been caused by drunk driving.

Are Police Reports Public Records
Many types of police records are exempt from public disclosure. There are two main reasons why they arent publicly available. First, disclosing the information could undermine an ongoing investigation. Second, it could jeopardize someones privacy and safety. In some cases, departments will release certain information related to the report, such as to a reporter doing a story. However, they rarely release a full copy.

CLUE stands for comprehensive loss underwriting exchange. This is a report generated by an agency called LexisNexis that gives an overview of your claims history. These reports contain information about the auto and property insurance claims that you have filed, usually dating back about seven years. Your insurance company will report relevant information to LexisNexis to generate the report. Any time you make a claim, CLUE will report information about the type of claim, the amount the insurance company paid, and the specific vehicle involved. Your CLUE report also contains basic information about you, such as your name, birth date, and policy number.

If you elect NOT to use the portal, requests may still be submitted using U.S. Mail and email using the Incident Report Request Form. However, those requests will be manually entered and tracked in the Incident Report Request Portal by a member of CISU. You do NOT need to complete the Incident Report Request form if you submit your request via the portal.

What Can I Do If My Clue Report Has Many Claims
If your CLUE report has an extensive number of claims on it, you may be worried about finding affordable car insurance. If youve made many claims in the past, many car insurance companies will be dubious about providing you coverage, because theyll consider you to be more of a risk. If your CLUE report is ridden with claims, it may be hard for you to get car insurance. However, there are some things you can do that will make the process easier. The first thing to do is review your CLUE report and make sure all the claims are accurate. Consider adding explanations to any claims that you believe will help.
Keep in mind that your CLUE report isnt permanent it only focuses on the last seven years. If you continue to drive safely and avoid claims, you could end up with a clean CLUE report in just a few years. Typically, car insurance companies will take recent claims more seriously than older claims as well. Focus on driving safely and avoiding behavior that could put you or your vehicle at risk. The more you can prove you are a safe driver to your insurance company, the less youll have to pay in claims.

How To Access Your Police
Members of the public have the right to request records containing their personal information from the Ontario Provincial Police or a municipal police service. Individuals who make these requests may have, for example, played a role in a suspected offence, been involved in a call for service or some other police-attended incident or had a relative who died in circumstances that led to the involvement of a police service.
Someone else, such as a lawyer or other person acting on your behalf, may make the request for you, or you may do so yourself. You may file a request by submitting a form and paying a fee, either by mail or in person.
Commonly requested records include:
- officers notes and
- police-related 911 calls.
Many people think that they cannot request a police officers notes, but a police notebook is generally in the custody or control of the police service and is subject to freedom of information requests.
Requesting information from a police service follows the same process as asking for records from other types of government institutions. However, the nature of police-held information means that frequently some of these records or some passages within these records are exempt from disclosure under Ontarios access to information legislation.
In all cases, if you disagree with a police services decision to deny your access request in whole or part, you may file an appeal with our office.

How Do Insurance Companies Use Clue Report
When you request a car insurance quote or apply for a policy, car insurance companies will request a copy of your CLUE report from LexisNexis. Theyll be able to see how many claims youve filed in the past, giving them an idea of your risk level as a customer. They can then use this information to determine how much they want to charge you. Research has shown that looking at past insurance claims is actually a good indicator of whether or not you will make insurance claims in the future.
Once you have a policy set up with your insurance company, they will report any claims that you file to LexisNexis. They do this once per month for all of their customers. Even if your insurance company denies a claim, it will still go on your CLUE report. LexisNexis tells insurance companies not to report instances where you call to ask about your policy or deductible since no actual claim was filed.
