If you receive an attend letter after submitting your application, follow instructions.

Individuals are requested to follow-up and attend the Customer Service Unit for a variety of reasons. Follow-up requests may be related to:

  • submission of fingerprints to confirm or eliminate a criminal record associated to an applicants information
  • Failure to submit a self-declaration form if you have existing criminal conviction
  • submission of fingerprints for the Vulnerable Sector search to confirm or eliminate a pardon associated to applicants information.
  • A vulnerable sector check is conducted as part of a criminal record check. It is a search of records suspended sexual offences. The vulnerable sector check is national RCMP policy and requires applicants who have a combination of name and/or gender and/or date of birth as a pardoned/suspended sex offender to undergo fingerprinting to verify their identity. The vast majority of those fingerprinted will not have a pardoned/suspended sexual offence however fingerprinting remains a necessity as it eliminates the possibility of an offender changing their name to circumvent a criminal record. This is not an accusation of criminal activity or guilt fingerprints are used to confirm your identity only. Please see the RCMP website for more information.

Who May Request A Copy Of A Police Report

Crime and Incident Reports

  • Insurance carrier against which a claim has been made and/or might be made
  • Person suffering bodily injury, property damage or loss

Not released:

  • If the disclosure would endanger the safety of a witness or other involved party
  • If disclosure could endanger the successful completion of an investigation and/or related investigation
  • Name and address of a victim of certain specified crimes will not be disclosed
  • If the Department is prohibited by law from releasing the report

Traffic Collision Reports

  • Drivers involved in the collision
  • Registered owners of vehicles involved
  • Parents of a minor driver involved
  • Passengers involved listed in the report
  • Insurance companies
  • Those incurring property damage as a result of the accident
  • Those who may incur civil liability as a result of the accident
  • Any attorney who declares under penalty of perjury that he or she represents any of the above persons

Juvenile Reports

Released to:

  • Parents of a juvenile may receive a copy of certain types of reports if their child is the only juvenile identified in the report.

When And How To Make A Police Report

There are many situations in which it is important to know how to make a police report. You should always file one for any incident or disturbance that affects you or your property such as a robbery, vandalism or attack.

When it comes to vehicle property, it is your right and responsibility to inform the police if anything happens to it, especially on public property.

If someone steals any car valuables such as your license plate, registration card or tires, you are advised to file a police report as soon as possible. If you fail to do so, you may be enabling further crime.

Your can become the victim of theft if your car is stolen or broken into and burglarized. You should call your local police department immediately in regards to a stolen vehicle or any personal documents like car registration.

While there is no immediate deadline for these types incidents as there is for car accidents, to get your property back and prevent further crime, you should get a police report as soon as possible.

Request Copies Of Police Reports

Most requests will be filled within 10 working days. To get a Police Report, complete and submit aRequest for Police Report either by mail or in person to:

Reports are to be picked up in person during business hours Monday Friday 8 a.m.- 6 p.m., closed weekends and holidays, at the Hayward Police Department.

The following fees are charged as outlined in the Master Fee Schedule:

  • Vehicle Accident Reports – $16.00
  • All Other Report – $16.00
  • Clearance Letters – $43.00 (given only if the requestor has not been arrested in the City of Hayward, and has lived in Hayward for the past six months or longer
  • Towed Vehicle Release – $235.00

Under the guidelines of Government Code, Section 6254, some reports are not released in their entirety and others may not be released at all.

Copy Of A Police Report

The VPD Correspondence Unit can provide a copy of a police report to you if you have reported or been involved in a property offence incident, such as:

  • break-and-enter
  • motor vehicle collisions where a police officer has completed an MV6020 report

With your consent, your insurance company can make this request on your behalf. In most cases, your police report number is sufficient for the VPD to find your file.

Police Report Vs Police Record

First, lets clarify something that is bound to cause some confusion: the difference between a police report and a police record. While these two items have similar names, they are in fact very different. A police report is a document filed by a police officer that essentially offers a summary of an arrest, incident, or accident. A police record, on the other hand, is usually the same as a criminal record and it includes a list and summary of an individuals history of interactions with law enforcement, including arrests and convictions.

Another point that deserves clarification is the difference between a police report and an accident report. In fact, theres not really a difference between these two items. An accident report is simply a type of police report that details the circumstances of a traffic accident. A police report, meanwhile, can include accident reports, but they cover a much broader range of incidents that may involve the police, including crimes. Note that some police departments call police reports incident reports. Remember that some accident reports may also be criminal reports, such as if the accident may have been caused by drunk driving.

Requesting A Police Report In Person

1. Gather the required materials.

Bring with you a form of identification like a drivers license or passport. If you are making the request on behalf of someone else, you will need to have a notarized letter with you. Most agencies require a small request fee. If you were directly involved in the incident whether as a victim, witness, or defendant you may receive a verification slip from the authorities to let you know that a police report has been filed. Bring this slip with you to exchange for the police report.

2. Visit the Records and Identification Division of the police department.

The Records and Identification Division may have exclusive hours throughout the day that are not your typical business hours, so be sure to verify online. They will ask you for your information, as well as basic information about the case. Then, they will take your payment to cover the request fee.

3. Retrieve the copy of your police report.

In some areas, you may have to pick up a copy of the police report in person, but some agencies may mail it directly to you. Either way, expect to pay a per-page fee for the copy of the report. Reports typically cost a total of $50 to $150.

Please visit the Santa Monica Public Safety Facility to request a report. Upon submittal, the City, in accordance with Government Code Section 6256, has ten days to respond. How Do I Obtain A Police Report

In most cases the public portion of police reports can be obtained from the APD Incident Report Database. Please read the Incident Report Database instructions on how to obtain the report. Make sure to enter the case number exactly how it is shown in the instructions.

For information about purchasing a copy of the public portion of a police report contact the Austin Police Department Report Sales Unit at 974-5499. Reports can be purchased at the Main Police Headquarters, located at 715 E 8th Street. Reports are 10 cents a page and only the public portion is available for purchase from the report sales office. This is the same report that can be obtained from visiting the APD Incident Report Database. Report Sales new hours of operation are Monday through Friday, 8:30 a.m. – 4 p.m.

How Much Will It Cost How Can I Pay For It

The cost of a report is listed below with the exception of a Domestic Violence report which is released at no cost to the victim pursuant to Family Code section 6228. If your request is approved, you will be contacted to arrange pick-up and payment of the report. If your request is denied, you will receive a written denial in accordance with Government Code section 6255.

The cost to obtain a police or EMS incident report is $17.00.

The cost to obtain a police arrest report is $25.00.

Traffic accident reports have a separate fee structure:

Hard Copy Police Reports Available

Reports are typically available for pickup four business days after the report was taken. To get the status of a police report, please call 770.513.5000.

Incident and accident reports may be obtained at any of the 7 locations listed below. Reports are available Monday through Friday. Offices/Precincts are closed on county holidays. Please refer to the website calendar section for a complete list of holidays. Accident reports are $5, incident report costs average 10 cents per page and in certain instances may be free to victims . On average incident reports run 50 cents or less and cash payment is preferred if under $1.00. Please bring exact change as we are often unable to break bills larger than $10.

How Do I Obtain A Copy Of The Report From State Police

If the officer who responded to the accident was a member of the Illinois State Police , you could request a copy of the crash report from the Official ISP Crash Report website. You should be prepared to provide details such as:

  • Drivers first and last name
  • Date of the crash
  • Location of the crash
  • Agency Crash Report Number . The responding officer should have given you a motorist report.

After entering this information, you can choose to have the accident report sent to you by email or by mail. You will have to pay a $5 crash report fee before your request can be fulfilled.

Police Incident Report Copy Request Forms

If you elect NOT to use the portal, requests may still be submitted using U.S. Mail and email using the Incident Report Request Form. However, those requests will be manually entered and tracked in the Incident Report Request Portal by a member of CISU. What We Know And Dont Know About Record Clearings Effects

Criminal justice researchers caution that even if automated record clearing expands to millions more people nationwide, it might not inevitably make it easier for people with criminal records to find jobs or places to live. How Do I Get A Copy Of My Car Accident Report In New York

After a car accident, youll need a copy of your accident report to file a claim for injuries and property damage. You and your attorney can obtain a copy of your car accident report by contacting the local police agency where the accident occurred and following their protocol. The agency will tell you whether the report is available and how much the fee is to retrieve it.

If the DMV has processed the accident report, you can access your certified accident report online or via mail. Accident reports become publicly available from 1460 days after the report was filed.
