Please be advised that the results for Level 1 Criminal Record Checks and Level 2 Criminal Record and Judicial Matters Checks are emailed to the applicant in a digital PDF form. Should you require a paper copy, please indicate this by writing HARD COPY in the Reason for Request area of the application, and a hard copy will be sent via post to your mailing address.

The Toronto Police Service provides three types of police reference checks, commonly known as background checks:

  • Criminal Record Check, which indicates any criminal convictions
  • Criminal Record and Judicial Matters Check, which indicates any criminal convictions as well as any outstanding entries and charges
  • Vulnerable Sector Screening, which is intended for individuals seeking employment or volunteer opportunities with vulnerable persons. This search indicates any criminal convictions, outstanding charges or warrants and certain pardoned offences.

If you’re unsure of what type of check you require, contact the agency/organization requesting the check so they can clarify. The Toronto Police Service does not make the decision on behalf of the organization.

Police Incident Report Copy Request Forms

If you elect NOT to use the portal, requests may still be submitted using U.S. Mail and email using the Incident Report Request Form. However, those requests will be manually entered and tracked in the Incident Report Request Portal by a member of CISU. You do NOT need to complete the Incident Report Request form if you submit your request via the portal.

Our request forms are available in several languages. Select your language, download and fill out the request form in your preferred language. The completed form may be either mailed or emailed to SFPD for processing. Instructions are included on all forms.

What Happens After I File A Report Online

  • Upon submitting your completed report online, you will see the words Your online police report has been submitted indicating your report is complete.
  • You will receive an email with a temporary case number until police have reviewed the report. Check your junk email folder if you do not receive the email in your inbox.
  • Once a police officer has approved the online report, you will be notified and provided a permanent case number.
  • The accuracy of the information you provide in the report greatly assists Peel Regional Police Service in investigating your incident. Please ensure the report is as complete and concise as possible.

Why Are Some Things Blacked Out On The Page

This is called redacting and it is done to protect the privacy of the people involved in the incident.

The Department redacts the names of all victims, all non-involved parties, all juveniles/minors and all suspects unless they have been booked into jail. This is a major reason why it takes time to prepare crime reports for online posting.

Once a suspect has been booked into jail, their name is no longer private. If they were booked into King County Jail you can view suspect information online at the King County Jail Register.

Identify Which Type Of Record Check You Need

During your online application, you will be required to select the type of police record check you need. The agency/organization requesting the check will communicate to you which type of check you need. The OPP cannot make this decision for you.

Police Department Request Form

If you cannot submit a police report request online or through the mail, then you will have to visit the police department in person and file a request there. Call beforehand since some departments only allow in-person requests on certain days. You can find your local police department here. Then you can find out about filling out a Police Department Request Form in person and how to get the information you need. Typically these police department request forms can be fulfilled that day.

A request will typically require you to provide your personal and contact information as well as details about the incident, including the type of incident, date, and who else was involved. Be aware that if the report pertains to an ongoing investigation then your request could be denied. If you are unclear about why your request was denied you should talk to an attorney.

All applicants will be fingerprinted and/or photographed to obtain Certificates of Conduct or Non-Criminal Fingerprint Cards.In order to decrease the spread of illnesses, face masks must be worn inside of the building. Applicants will be required to take a temperature reading upon entering the building. To allow for social distancing, do not bring additional people to your appointment. Translation services will be provided, if necessary. When planning your travel time, include extra time to pass through two security checks.

All Certificates of Conduct will be mailed via United States Postal Service after the completion of processing. Processing and mailing should occur within ten business days.

Verification of Incident reports are provided by the Criminal Records Section, which stores and maintains crime and lost property reports and provides information from these files to members of the public and authorized agencies, as required by law and department regulations.

You can request a copy of a Verification of Incident by submitting your request online or by mail. The form is available for , or can be picked up at any police precinct, Housing Bureau public service area, or Transit Bureau district. Reports can only be returned through the mail and are NOT available at Police Headquarters.

If mailed, you must include two copies of the completed form, along with a self-addressed stamped envelope. Complainants/victims designating an authorized representative must complete, notarize, and mail the request to the address above.

Step : Make Sure Your Occurrence Can Be Reported Online

Use the information below to see if you can use this system. If your occurrence qualifies for online reporting, proceed to the next step. If your occurrence does not qualify for online reporting, it cannot be accepted by the officer managing online reporting. These occurrences require contact with a local OPP officer for investigation. Please call the OPP toll-free at 1-888-310-1122 to file a report. Links to external websites including those with video, digital images or documents that are not supported by this application cannot be managed through this online reporting service. Please include a narrative description of the occurrence if you will be filing an online report. Filing a false police report is a criminal offence.

Upon Completion Of This Report Process You Will:

  • See the words: “Your online police report has been submitted”.
  • You will then immediately receive a temporary police report file number and an email informing you that we have received your report and that it is pending review. A temporary file number will appear in this format: T11000999. Temporary file numbers cannot be used for insurance purposes.
  • If you do not receive an email with a temporary file number, please check your Spam or Junk-Email folder.
  • A police officer will review your report and either Approve, Reject or request additional follow-up information from you. You will be advised via email of the status of your report. Do not reply via email to any of these automated messages as these accounts are not monitored. Follow the directions provided on how to respond regarding your report.
  • Once your report is approved, you will receive a permanent report number – this number appears in this format: 11-123456.
  • Be able to print a copy of the police report to keep for your records only. This copy cannot be used for insurance purposes.

Filing A Police Report Online

This system allows you to submit a report from your computer or mobile device without going to an OPP detachment. All submissions will be sent directly to the OPP and reviewed by an officer. You may be contacted to provide more information. You must have a valid e-mail address in order to submit a report.

    How Do I Pick Up My Check

    Once payment has been confirmed and the application has been submitted, a Customer Service Representative will begin to complete the request. The applicant will receive an email to attend the police facility when the check has been completed.

    When picking up the check, the applicant must provide identification .

A station clerk or officer may be able to locate the status of your report with the date/time and location of the incident. Not having your report or incident number can also delay receipt of your report when requesting a copy. Sometimes police reports are delayed because the officer has yet to process the paperwork, or there is a mistake with the paperwork that must be corrected before generating a report.     Where To Find Police Records

    First off, if you are or were a criminal and have a criminal record, you are entitled to a copy of that record thanks to the Freedom of Information Act. As stated earlier, police records are a matter of public record, therefore they are available for free access to the public. The Freedom of Information Act states that any person has the right to request access to federal agency records or information, except for exemptions that are listed. This means that anyone in the U.S. can request basic information that is considered public record, such as voter’s registration, property deeds, or criminal records.

    The U.S. system of recording and storing these police records isn’t always the easiest to navigate. Records can have issues crossing state lines, which is why it’s important to find a database that checks all states and compiles the findings. A lot of people say the U.S. should make it easier and have all records in one pool, but that’s just not how things are done.
